Community Health Access Program (CHAP)
The Community Health Access Program (CHAP) is a nurse-managed clinic that provides low-income individuals and families with access to health care. The emphasis is on health education, disease prevention, wellness monitoring and primary health care. To qualify for CHAP services, clients must earn less than 185% of poverty level and not have health insurance including Medicaid or Medicare. CHAP is located at 333 E. Miller Drive in . Walk-ins welcome.

Well Child Assessments
The Well Child Program provides immunizations, health screenings and physical assessments for children up to age 18. The emphasis is on making sure that children grow and develop normally. Fee: Sliding fee scale

Parish Nurse Program
The Parish Nurse Program is a heath care ministry initiated by Hospital in cooperation with area faith communities. Nurses chosen from within or outside the congregation utilize the physical, emotional and spiritual dimensions of nursing practice to help transform the congregation into a place of wellness and healing. Parish nurses volunteer from five to 15 hours a week to provide health and wellness education, individual and family health counseling, and serve as a community resource for various health care needs of church members. There are currently eight area congregations participating in the program. Nurses from these congregations gather monthly for networking and fellowship.

Assisted Medical Transport Service
This service offered by the Hospital Ambulance Service serves the special transportation needs of people in wheelchairs or those with other mobility problems who experience difficulty traveling to and from health care services. You may schedule trips in Monroe and surrounding counties to and from physician and dentist offices, pharmacies, treatment sites such as dialysis or radiation therapy centers, Hospital and other area hospitals. The specially-equipped vans are staffed by -certified emergency medical personnel.
Monday-Saturday, 6 a.m.-8 p.m.
Fee: $20 one-way trips for walk-on clients and dialysis clients, and $30 wheelchair clients. Mileage rate is $1.50/mile in Monroe County, $2.00/mile outside Monroe County. Rates are for private billing. Medicaid will not pay for the first 10 miles of any trip.
